ARE YOU A NEW PATIENT?
We understand that you may be apprehensive about your first physical therapy appointment, but from the moment you set foot in our state of the art clinic, you’ll know you made the right choice for your rehabilitation needs. Our welcoming front office staff members are happy to help you begin your path to recovery and answer any insurance, scheduling, or billing questions along the way.
WHEN SHOULD I ARRIVE?
Please arrive 30 minutes prior to your scheduled appointment to allow yourself to complete all of the necessary patient paperwork and for us to answer any questions you may have. To save you time, please print and complete the new patient paperwork below, prior to your first appointment.
WHAT SHOULD I WEAR & BRING?
Please wear loose clothing to your appointments, such as athletic wear, and please do not wear jewelry. Please bring the following with you to your appointment:
- A prescription/referral for physical therapy from your healthcare provider
- Completed New Patient Forms (download below)
- Photo identification (valid driver’s license, valid state ID, school ID card, or valid passport)
- Insurance card(s)
- List of current medications
- Any surgical reports associated with your diagnosis
INSURANCE
We have established relationships with every major insurance providers and will also accept assignment for any insurance provider with out-of-network benefits.
